Inpathy is a behavioral practice that provides psychiatry and individual therapy services.

Our self-scheduling services are appropriate for individuals who are seeking behavioral health treatment in an outpatient setting and who:

• Are NOT currently experiencing active suicidal thoughts, hallucinations, thoughts to harm themselves or others.
• Do not need medication for anxiety or adult ADHD
• Have not had more than one (1) psychiatric inpatient hospitalization or Intensive outpatient service in the last six (6) months.

By continuing and scheduling an appointment, you are agreeing you have read and understand the above guidelines and do fall into any of the above categories.  

Inpathy Systems TERMS AND CONDITIONS OF USE

Your access to Inpathy services is subject to and conditioned upon your agreement to these Terms, our Privacy Policy, and all applicable laws and regulations governing access to the Inpathy Systems and services. By agreeing to the following terms and conditions, you agree to these Terms below. Other areas or particular aspects of the services may be subject to and conditioned upon your compliance with additional terms and conditions (e.g. consents, etc.), which will be presented to you prior to your access of those areas or services.

Use and Services.

I am requesting access to portions of my health information and the ability to communicate with my Inpathy health care team concerning my health information via the Internet using an electronic application used by Inpathy.

I understand that by accessing or using Inpathy Systems on behalf of another person, I represent that I am authorized to accept these Terms on that person’s behalf and that the person to be responsible to us if myself or the other person violates these Terms.

Technology Requirements.

I understand that the Inpathy system is intended as a secure online source of confidential medical information. I must have internet access and my computer's internet browser must be either:
Mozilla Firefox 52.x and above
Safari 9+ on Mac
Google Chrome 52.x and above

I agree to be responsible for any telephone charges and/or Internet service fees incurred by accessing the Inpathy Systems.

I understand that Inpathy System is never to be used as a means of communication to health care providers for urgent or emergency matters. For all emergency matters that I believe might immediately affect my health or well-being, I will dial 911 without delay and/or go to the emergency department of a local hospital.
Privacy and Security.
I understand that my Inpathy health care team may send me messages via the Inpathy System. These messages may contain information regarding my health and medical care. It is my responsibility to monitor these messages that are sent to my Account Inbox. I agree not to hold Inpathy liable for any loss, injury or claims of any kind resulting from messages that I fail to read or act upon in a timely manner.

I have the option of entering a valid and functional email address that may be used to notify me of the presence of messages in my consumer portal. If I opt to provide this email address for notification of the presence of messages, I will update any changes of my email address as needed to provide timely delivery of messages.

I agree that it is my responsibility to select a confidential password, to maintain my password in a secure manner, and to change my password if I believe it may have been compromised in any way. If I share my Inpathy ID and password with another person, that person may be able to view my health information, and information about someone who has authorized me as an Inpathy proxy. I agree not to hold Inpathy or any of its subsidiaries liable for any loss, injury or claims of any kind resulting from my disclosure of the confidential password or confidential information on the Inpathy system.

I understand that the Inpathy system contains selected, limited medical information from a patient's medical record and that the Inpathy portal does not reflect the complete contents of the medical record.

I understand that my activities within Inpathy may be tracked by computer audit and that entries I make may become part of the electronic record.

Appropriate Use of the Inpathy System.
I understand that access to the Inpathy system is provided by Inpathy as a convenience and that Inpathy has the right to deactivate or restrict access to this system at any time for any reason or no reason without notice to me.

I understand that my use of Inpathy must be appropriate and may not be used to harass, intimidate, defame, or threaten any individual or for any illegal activity and agree to use Inpathy Systems in accordance with all applicable local, state, and national regulations

I understand that use of the Inpathy system is voluntary and I am not required to use Inpathy or to authorize a Inpathy proxy.

Disclaimer.
I understand that the Inpathy system may not be available to me all the time due to system failures, back-up procedures, periodic system maintenance, or other causes beyond the control of Inpathy. Access is provided on an "as-is, as-available" basis and does not guarantee that I will be able to access the Inpathy system at any particular time.

The Inpathy Systems may include text, graphics, images, trademarks, trade names, service marks, logos, and other content therein (collectively, the “Inpathy Systems Content”) that are owned by or licensed to Inpathy and are protected under both United States and foreign laws. Except as explicitly stated in these Terms, Inpathy and our licensors reserve all rights in and to our Inpathy Systems Content.

I UNDERSTAND THAT INPATHY TAKES NO RESPONSIBILITY FOR AND DISCLAIMS ANY AND ALL LIABILITY ARISING FROM ANY SOFTWARE INACCURACIES OR SOFTWARE DEFECTS, COMMUNICATION LINES, VIRTUAL PRIVATE NETWORK, THE INTERNET OR MY INTERNET SERVICE PROVIDER (ISP), ACCESS SYSTEM, COMPUTER HARDWARE OR SOFTWARE, OR ANY OTHER SERVICE OR DEVICE THAT I USE TO ACCESS THE INPATHY SYSTEM.

Miscellaneous.
During times when the Inpathy system is unavailable, other communication methods (e.g., telephone) should be used to access Inpathy.

We reserve the right to change our Terms at any time. Your continued use of the Inpathy Systems will constitute your agreement to the changes we have made. The last date these Terms were revised is set forth at the top of this document.
